If you didn't receive either of these emails (perhaps because your high school email account was disabled), do NOT worry since an exact copy of the email is posted above. All important information from the Office of Student Affairs is posted on the web pages referenced below and on the New Students web site. All future communication from our office will go to your Amherst email account.
Please review the following guidelines and instructions for getting started. These will help you make a smooth transition into the new semester. In particular pay attention to sections with deadlines.

PDF and Web Forms
Health Forms - Deadline: July 5, 2014
The Health Forms are in the Portable Document Format (PDF), and you must print, complete, and return these forms to Health Services via mail, fax, or as email attachments. These forms are required components of your pre-registration medical file at Amherst College. Massachusetts Law requires up-to-date immunizations or blood tests verifying immunity.
Accessibility & Accommodation Disclosure and Request Form - Deadline: June 15, 2014
This form should be completed when a student has indicated his or her desire to request reasonable accommodations from the college for a learning or physical disability.
