Required Health Survey & Training

Faculty and staff working on campus must complete the Health Status Survey every day before coming to work, and must complete the online training at least once per semester.

Symptom Monitoring Requirement

Faculty and staff returning to work on campus must conduct daily symptom self-monitoring for COVID-19 before reporting to work each day. You must be free of any flu-like symptoms for a minimum of 24 hours without the aid of medication, including over-the-counter pain relief, before returning to work on campus. Signs and symptoms may include:

  • breathing difficulty
  • cough
  • fatigue
  • fever or chills
  • muscle aches and pains.

Faculty and staff should consult with their primary health care provider if they experience any of these symptoms or if they feel ill.

Persons 65 years or older and people of any age who are immuno-compromised (serious underlying medical conditions) might be at higher risk for severe illness from COVID-19. Serious underlying medical conditions may include, but are not limited to:

  • asthma
  • cancer
  • diabetes
  • heart disease
  • kidney or liver disease
  • respiratory illnesses
  • weakened immune systems.

Online COVID Screening Tool

Every faculty and staff member returning to work on campus is responsible for daily self-screening, including a temperature check, which must be conducted prior to arriving on campus each day and reported using an online tool provided by IT: Go to the Health Status Survey.

Anyone displaying a fever after arriving on campus should leave campus immediately.

Daily Screening Questions

1. Since your last day of work, or last visit here, have you had any of these symptoms?

  • cough
  • shortness of breath or difficulty breathing

...or at least two of these symptoms:

  • fever
  • chills
  • repeated shaking with chills
  • muscle pain
  • headache
  • sore throat
  • new loss of taste or smell.

2. In the last 24 hours, have you had any signs of fever or a measured temperature above 100.3 degrees?

3. Have you had any close contact with a person diagnosed with COVID–19?

NOTE: “Close contact” includes living in a household with a person or caring for a person who has tested positive for the disease; being within six feet of a person who tested positive, for 15 minutes or longer; or coming in contact with the secretions of a person who tested positive (being coughed or sneezed on, using shared utensils, etc.).
