Join the Benefits Team to:
- gather information during a general overview
- discuss any benefits issues or changes you may be contemplating
- complete the Sliding Scale Cost Sharing application. Note: Applicants will need to provide their amount reported on Line 9, Total Income, on Form 1040 U.S. Individual Income Tax Return (or Total Income on other tax return forms). The Sliding Scale Cost Sharing application needs to be completed annually.
