Meet independently with a member of the Office of Human Resources staff to verify household income for Sliding Scale Cost Sharing for Open Enrollment 2021. Household income for this purpose is defined as the amount reported on Line 9, Total Income, on Form 1040 U.S. Individual Income Tax Return (or Total Income on other tax return forms). Please bring a copy of the applicable 2020 U.S. Individual Income Tax Return(s). The Sliding Scale Cost Sharing application needs to be completed annually.
